Identification of Occurrence:

Failure to analyze Dose Equivalent Iodine 131 upon a power change greater than 15%

within a one hour period contrary to Table 4.4-8 of Technical Specification 4.4.8.

Conditions Prior to Occurrence:

Mode 1 power operations.

Description of Occurrence:

At 1130 when reviewing turnover from the previous shift, the chief Chem / Rad technician discovered that the DE-I 131 was not analyzed upon a power change of 30%

within one hour. The Chem / Rad personnel were not notified of the power escalation as required by the special condition surveillance plan SP-442. The analysis was immediately initiated and analyses were made upon subsequent power changes, substantiating the DE-I 131 remained within acceptable limits.

g Designation of Apparent Cause:

The cause of this occurrence is attributed to procedural inadequacies in that the Chem / Rad personnel were not notified of power escalation.

Analysis of Occurrence:

During power escalations the Iodine spiking phenomena can cause DE-I 131 to exceed the allowable Tech Spec limits.

Corrective Action:

Changes to the operating procedures dealing with power changes have been initiated to insure notification of Cham / Rad personnel upon power level changes greater than 15%/ hour.

This is the first occurrence of this event.
