Dear Mr. Engelken:

!!r. Crews' letter of October 28, 1976, which cocumented an inspection conducted by Mr. R.T. Dodds of the Office of Inspece.1on and Enforcement on October 4 - 9, 1976, identifies as an item of infraculon under the Notice of Violation, the failure to verify the decay heat removal system leakage rate specification prior to reactor operation c. August 29-30, 1976.

As previously reported to your of fice under Reportable Occurrence 76-13 of October 18, 1976, the leakage of the "B" Decay Heat Pump was

'i discovered on August 22, 1976, during the refueling interval surveillance

\\f check of leakage on the Decay Heat System. The system was immediately depressurized, the surveillance test stopped and the leaking pump seal repaired.

On August 29 a surveillance test of the pump was conducted during which time the pump ran for 37 minutes and the visual inspection verified that there was no leakage from the pump.

Upon completion of the surveillance test to check operability of the pump and with an observed zero leakage from the pump, the Decay Heat System was put into operation. The refueling interval surveillance for actually measuring the "B" Decay Heat Loop leakage was e-run on September 1, the results of which showed that the entire "B"

Loop had a leak rate of 0.0185 gallons / hour, well below the specification limit of 0.63 gallons / hour.

The supervisory level operating personnel have been made aware of the requirement that a surveillance test must be completed prior to declaring the system operable if the initial surveillance test was determined to be unsatisfactory.
