Gentlemen:
This refers to the investigation conducted by Messrs. L. J. Garvin and
otner members of tnis office during the period October 21 to November 11,
1976 of activities authorized by NRC License ilo. DPR-54, and to the
discussion of our findings held by Messrs. Spencer and Garvin with Mr.
D. C. Browning and other members of your staff on November 17, 1976.

Areas examined during this investigation dealt with allegations of poor
workmanship and of falsification of records associated with the fabrica-
tion of the replacement fuel storage racks for Rancho Seco Unit No.1.
Within this area, the investigation consisted of testing of welds,
examination of procedures and records and interviews with SMUD and
vendor personnel.
During this investigation it was found that certain of your activities
appear to deviate from your December 19, 1975 request for a license
amendment. These items and references to the comitments are listed in
Appendix A to this letter.
Please provide us within 20 days, in writing,
with your comments concerning these items, a description of any steps
tnat have been or will be taken to correct them and the date that all
corrective actions were or will be completed.
Also, since the spot
welds were not tested as described in your design technical specifica-
tion, please evaluate the effect of the decreased testing on the in-
tegrity of the spot welds and include the evaluation in your reply to
this letter.
In addition to the need for corrective action regarding these specific
deficiencies, we are concerned about the implementation of your quality
-
assurance program that permitted these deficiencies to occur.

sequently, in your reply, you should describe in particular, those

actions taken or planned to improve the effectiveness of your quality
assurance program.
In accordance with Section 2.790 of the HRC's " Rules of Practice,"
. Part 2, Title 10, Code of Federal Regulations, a copy of this letter and
the enclosed investigation report will be placed in the NRC's Public
Document Room.
If tnis report contains any information that you believe
to be proprietary, it is necessary that you submit a written application
to this _ office, within 20 days of the date of this letter, requesting
tnat such information be withheld from public disclosure.
The applica-
tion must include a full statement of the reasons why it is claimed that-
the information is proprietary. The application should be prepared so
that any proprietary information identified is contained in an enclosure
to tne application, since the application without the enclosure will
also be placed in the Public Document Room.
If we do not hear from you
in tnis regard within the specified period, the report will be placed in
the Public vocument Room.
