the r=f==14=g water storage tanka into uncontrolled water pathways.

In one case, failure of the water level indfemtor is tho' control room during a f4114== operation rossited in liquid overflow from the
- refueling water storage tank. to the facade drain tror gh which drained to' the facada sump and discharged to the sewage treatment plant and to the sausae_rarention pond. In tha second case, operator error resultad in liquid overflow from the refueling water storage tank to 3
the plant dr=i==g= system to tha stora dr=1 mate system and subsequently f
to Black Creek. A copy of the licensees' reports on these two

occurrences is enclosed for your information.
I As a result of these occurrences, we regeest that you perform a review l
of the refueling water storage tank system design, and other liquid tanks which contain radioactivity, that overflow to other than a l
controlled vater pathway to the envirees. All modes of operation i
and failure must be cassidered, In the cases cited above, alarms for
}
high level overflow or areammive lankage had not been i=-1=M for the r==1r = and control of such overflow or laakage had not been adequately considered in the design of these systems.
l Also, your procedures should be reviewed to ensure that they provide for operator surveillance and proper action concerning the liquid storage tanks which contain radioactivity, particularly when 1.luid la f
