Facility: Oconee Nuclear Station, Seneca, South Carolina Identification of Occurrence: Failure to Verify Keowee Hydro Unit 2 Operability Conditions Prior to Occurrence: Unit 1 100% FP '

Unit 2 100% FP Unit 3 100% FP Description of Occurrence:

On March 23, 1978 at 0749 Keowee Hydro Unit 1 was removed from service for planned breaker maintenance in accordance with procedure MP/0/A/

2001/2. Keowee Unit 2, the standby unit, was verified operable at 0722 and at 1544. The verification at 0722 satisfied the "erification with-in one hour requirement of Specification 3.7.26b, but the reverification at 1544 failed to meet the requirement to verify the standby Keowee unit operable within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

Apparent Cause of Occurrence:

The cause of this occurrence was a personnel error. Personnel failed to properly use the station procedure " Removal and Restoration of Equip-ment" to document the testing requirements when Keowee Unit 1 was removed from service.

Analysis of Occurrence:

Keowee Unit 2 was verified operable at 1544. Additional sources of off-site power as well as Keowee Unit 2 were available to provide emergency power for the Oconee Station if it had been required. Thus, the health and safety of the public were not endangered by this incident.

l Corrective Action: l Operating personnel concerned with this incident were reviewed by the l Operating Engineer to assure that they fully understood the required l actions.

The procedure for removal of a Keowee Unit from service has been revised to include a checklist to assure that the requirements of Technical Spe-cification 3.7.2 are complied with.
