Dear Mr. Moseley:
This letter supplements our reaponse to OIE Inspection Report 50-269,
-270, -287/77-3 as submitted by our letter dated May 2, 1977.
With regard to Item LA, in addition to the corrective action stated, Station Directive 3.3.5 will be revised by May 25, 1977 to require that a cognizant supervisor review and approve all changes made or mistakes corrected in a work request. This action will provide additional assurance that maintenance and repairs are performed in accordance with an approved work request.
With regard to Item 1B, in addition to the coordination problem noted in our response, Health Physics personnel did not adequately adhere to the procedures controlling the proper disposal of radioactively contaminated trash and the proper tagging of items removed from the Radiation Control Zone.
It is felt that Health Physics procedures for Radioactive Waste Disposal and Removal of Items from Radiation Control Zone of Radiation Control Area, are adequate to control those problems identified in Item 13.
Therefore, the following administrative action, as stated in our May 2, 1977 letter, was taken.
The item has been reviewed with all personnel in the surveillance unit of Health Physics to assure that they understand how radioactive waste should be handled under similar circumstances.
In addition, the entire Health Physics section is in the process of reviewing all health physics practices and procedures to verify understanding of the procedures and their applicability to the work being done. It is anticipated that this action will prevent future occurrences of this type.
