APPENDIX A NOTICE OF VIOLATION Texas Instruments, Incorporated Docket No. 70-33 Based on the results of an NRC inspection conducted on March 12-14, 1980, it appears that one of your activities was not conducted in full compliance with the conditions of your license as indicated below.
Item A is a deficiency.

Condition No. 8 of Special Nuclear Material License No. SNM-23 incorporates the licensee's Health and Safety Manual.
Section 5.2.3.5 of the Health and Safety Manual states records will be kept on all assignments, inspec-tions, repairs a.'d leak tests of respirators.
Contrary to the above, records were not maintained on respirator assign-ments and leak tests on May 15, 1979, May 17, 1979, and September 7, 1979.
