Extensive cracking discovered in the valve disc guide on all six valves.

Wyle Lab Technician during decontamination of valves prior to refurbish and reset of valves.

10. Corrective Action:

This is being determined by FPC, Dresser and Wyle Lab representatives at this time.

All other valves of this type in CR-3 systems will be subject to the same corrective action determined appropriate for the six (6) valves under current evaluation and will be accomplished prior to restart.
