--vent header deflector installation outage, local leak rate testing being, performed per HNP-2-3952, PRIMARY CONTAINMENT PERIODIC TYPE B

AND TYPE C

LEAKAGE

TESTS, showed dirty L -
radwaste isolation valve 2G11-F004 to be leaking in excess of specified criteria ("As Found" leakage would not pressurize).

This valve's leakage rate is included in both the.60La and

'the

.009La leakage rate limits of Tech Specs sections

% 3.6.1.2.b.1 and 3.6.1.2.b respectively.

This is a repetitive occurrence 1980-041.

- The cause of the unacceptable leakage rate for the valve was failure of the valve gate to properly seat.

Corrective valve
