j x-APPENDIX A NOTICE OF VIOLATION South Carolina Electric and Gas Company License No. CPPR-94 Virgil C. Summer Based on the NRC investigation September 10 - December 19, 1979, certain of your activities were apparently not conducted in full compliance with NRC require-ments as indicated below. These items have been categorized as described in correspondence to you dated December 31, 1974.

A.

As required by paragraph (a)(1) of 10 CFR 50.55a, " Structures, systems, and components shall be designed, fabricated, constructed and inspected to quality standards commensurate with the function to be performed." For safety-related piping this requirement is implemented by Table 3.2.1 of the FSAR which designates ASME Section III as an applicable quality standard.

As required by Criterion V of Appendix B to 10 CFR 50 and implemented by Section 17.1.5 of the FSAR, " Activities affecting quality shall be prescribed by documented instructions, procedures, and drawings... and shall be accomplished in accordance with these instructions, procedures, and drawings."

Contrary to the above, on October 30, 1975 che licensee's procedures did not include the fillet weld size requirements stipulated by ASME Section III for socket-welding flanges and an example weld (SW-4 on Iso. DE-CS-24)

examined did not comply with the ASME Section III requirement.

This is an infraction.

B.

As required by Criterion V of Appendix B to 10 CFR 50 and implemented by Section 17.1.5 of the FSAR, " Activities affecting quality shall be prescribed by documented instructions, procedures, and drawings... and shall be accomplished in accordance with these instructions, procedures, and drawings."

The minimum fillet weld size requirements for ASME Section III socket-welding piping components other than flanges are specified on welding technique sheets 1-1-B-11R2 and 8-8-B-11R3 as 1.09 x pipe wall thickness but not less than 1/8 inch.

Contrary to the above, on November 1, 1979, a number of ASME Section III welds has sizes below the specified minimum. Examples including SW-31 on f

Iso. DE-SI-04, SW-4 on Iso. DF-CS-02, FW-27 on Iso. DE-CS-24, FW-3 on Iso.

DE-EF-09, and SW-3 on Iso. DE-RC-08.

This is in infraction.
