Appendix A NOTICE OF VIOLATION Northern States Power Docket No. 50-283; 50-306 Company Based on the inspection conducted on January 2-31, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.

Item 1 is an infraction and is applicable to Unit 1 only.

Item 2 is an infraction and is applicable to both Units.

Technical Specification 3.3.B.1 states that a reactor shall not be made or maintained critical nor shall it be heated above 200*F unless the following conditions are satisfied... c. The spray additive tank contains not less than 1950 gallons of solution with a sodium hydroxide concentration of not less than 30% by weight.

This in. plies that the spray additive be available for addition to the containment spray system when actuated.

Contrary to the above, from about 2330 on January 3, 1980 to 0600 January 6, 1980, manual isolation valves CA-1-2 and CA-1-4 were closed, isolating the spray additive tank from the containment spray system.

Technical Specification 6.5 states that detailed written procedures, including the applicable checkoff lists and instructions, covering areas listed below shall be prepared and followed. These procedures and changes thereto, except as specified in TS 6.5.D., shall be re-viewed by the Operations Committee and approved by a member of plant management designated by the Plant Manager.

Integrated and system procedures for normal startup, operation and shutdown of the reactor and all systems and components involving nuclear _ safety of the facility.

C1.3, Unit Shutdown Procedure, controls actions for unit shutdown.

Step 37 of procedure C1.3 states in part:

When the RC temperature has been reduced to less than 200 F (180 F if pressurizer steam bubble is retained):

Remove safeguard hold card, close and attach SECURE cards to CA-1-2 [2CA-1-2], CV-31941, CV-31939] Outlet Isol and CA-1-4 [2CA-1-4], CV-31938 (CV-31940] Outlet Isol. Deliver safeguards hold card to the Shift Supervisor.

Contrary to the above, on January 3, 1980, while performing procedure C1.3 for Unit 2 shutdown, an operator erroneously closed valves CA-1-2 and CA-1-4 on Unit 1, isolating the containment spray additive tank from the containment spray system while Unit I was operating, and did not deliver the safeguards hold cards to the Shif t Supervisor.
