Dear Sir:

Attached is our response to a Wotice of Violation that resulted from an inspection of the irojan Nuclear Plant conducted by members of the Region V staff on January 21 through 23, 1980.

During this inspection, it wa., determined that two items at the plant were not conducted in full compliance with the NRC requirements.

Our response details the actf an taken to correct these infractions.

TROJAN NUCLEAR PLANT-1 RESPONSE TO NOTICE OF VIOLATION FINDING i -

Paragraph 5.a of Appendix A to 10 CFR 55 requires that ' records be main-tained to document the participation of each licensed operator and senior operator in the requalification program. Paragraph 7.1.7 of Trojan Train-ing Procedure TP-2-2, which is the licensee's NRC-approved operator retraining program, requires that records of reactivity control manipulations be maintained.

Contrary to these requirements, the licensee documented only one of the' ten or more reactivity control manipulations performed by one licensed 4

senior operator during his two-year license term ending in October, 1979.

PGE RESPONSE l

Reactivity manipulations are documented each shift by the use of an on-shif t i

training record.

It is the responsibility of each license holder who performs reactivity manipulations to enter or ensure entry of his manipulations when-ever they are completed. This record is then f orwarded to the Training Department for entry into the licensee's retra:.ning file.

4 The licensee involved was unfamiliar with the system for documetating reactivity manipulations; therefore, these evolutions were not documented in the retraining files.

The licensee had performed 14 reactivity manipu-2 lations during the term of his license.

documented them in acecrdance with the Licensee Retraining Program. A i

TROJAN NUCLEAR PLANT RESPONS" TO NOTICE OF DEVIATION FINDING Contrary to the licensee's commitments expressed in p'aragraphs 4.1.11 and 13.0, respectively, of Trojan Nuclear Plant Training Procedure TP-2-2 which is the licensee's NRC-approved operator retraining program, the following actions were not accomplished in the calendar year 1979:

1.

The Operations Supervisor did not review participation of licensed reactor operators or senior reactor operators in the reactivity control manipulations portion of the retraining program, 2.

The Training Supervisor did not prepare quarterly summaries of the retraining program (intended to include a general progress report on the retraining program and a listing of actual or potential difficulties with respect to meeting its requirements).

PCE RESPONSE TO ITDI 1 The review of reactivity manipulations by the Operations Supervisor was intended to ensure a variety of reactivity manipulations for each license holder.

During the extended shutdowns in 1978 and 1979, few reactivity manipulations were performed at Trojan and the reactivity manipulation program was augmented by assigning license holders to simulator sessions at the Westinghouse Nuclear Training Center.

During this period, the Operations Supervisor reviews were neglected.

All reactivity manipulations have now been reviewed by the Operations Supe rvisor. A Training Assistant has been assigned to follow reactivity manipulations and to schedule reviews of the records. Furthermore, the Licensee Retraining Program will be revised to reassign the requirement for review of the reactivity manipulations to the Training Department.

PGE RESPONSE TO ITD12 Annual summaries reviewing all licensed activities were issued at the end of each yearly retraining cycle. Additionally, a report of potential problem areas was included as part of the monthly retraining program. Due to these reports, the quarterly summaries by the Training Supervisor were neglected.

The Licensee Retraining Program will be revised to require annual vice quarterly reviews.
