This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2.b.(3).

Conditions Prior to Occurrence The plant was shutdown for a refueling / maintenance outage.

The reactor was subcritical.

The reactor mode switch was locked in refuel.

The cavity wcs flooded.

Description o? Occurrence On Thursday, February 28, 1980, during review of the Master Surveillance Schedule, it was discovered that Diesel Generator Weekly Battery Surveillance (636.2.005) and Main Station Weekly Battery Surveillance (634.2.002) were not performed during week No. 7 The test was scheduled to be performed on Friday, February 1,1980, but due to rescheduling of personnel for various outage jobs, the two surveillance tests were inadvertently overlooked.

This week was the only week that the tests were not performed.

Accarent Cause of Occurrence The apparent cause of the occurrence is personnel error and can be attributed to failure to adhere to the distributed surveillance schedule.

a Reportable Occurrence flo. 50-219/80-12/3L Page 2 March 27, 1980 Analysis of Occurrence The manufacturer's Instructions for battery care recommend that written records be kept to follow the performance of batteries.

This type'of surveillance has been demonstrated through experience to provide an Indication of a cell becoming irregular or unserviceable long before it becomes a failure. The batteries will deteriorate with time, but precipitous failure is unlikely.

The safety signi-ficance is considered minimal because there were no adverse effects noticed in the following surveillance test performed on each of the batteries.

Corrective Action

The importance of the surveillance is understood and the Master Surve.Illance Schedule will be adhered to in order to prevent this situation from recurring.

An evaluation will be made to determine ways of improving the execution of the surveillance test program.
