The attached report by our field inspector of a visit to the subject

facility on December 2-4, 1974 is forwarded for information.

.

We consider the violations identified during this inspection, when

coupled to those identified during the previous inspection, indicative

of a breakdown in the licensee's QA program as it relates to maintenance

activities. However, we feel our enforcement letter requesting the

licensee to address the effectiveness of his QA program is sufficient

at this time since our previous inspection ended only about two weeks

O

prior to this inspection, and the licensee had not yet received the

enforcement letter from our previous inspection.

,

Our inspectors believe the licensee is fully aware that the QA effort

for maintenance activities falls short of his program requirements from

his evaluation of conditions brought to his attention during our previous

inspection. Their impression during the management meeting was that the

licensee management intends to take immediate action to bring maintenance

activities into conformance with the stated program requirements. They

will examine the licensee's actions during the scheduled inspection in

January 1975.
