Dear Mr. Engelken:
In your letter dated December 27, 1974 you noted three (3) areas
}
fn which Rancho Seco Unit No.1 appears to be in violation of AEC regu-Q 1ations and our Quality Assurance Program.
This letter outlines the corrective steps taken to avoid further violations in the noted areas.
The corrective steps taken with respect to non-use of Mainten-ance Inspection Data Reports (MIDR's) are hs follows:
1.
Effective immediately all Class I items and selected Class II items will be, inspected using a Maintenance Inspection Data Report (MIDR) when required by Quality Assurance Pro-cedure (QAP) No. 6.
2.
5 avoid further violations in this area, QAP No. 6 has been revised to give better definition,as to when the MIDR is required.
In addition, the Quality Assurance Director has been directed to perform more frequent audits in this area to verify compliance.
3.
Full compliance, i.e. revising and release of QAP No. 6, plus utilization of MIDR's will be achieved within 30 days.
The corrective steps taken per item (2) which pertains to non-use of inspection stamps by personnel to provide uniform documentation of inspection findings are as follows:
1.
As noted in RO Inspection Report No. 74-12, stamps have been issued to designated inspectors in response to this enforce-(,/

To avoid further violations in this area, the Quality Assur-ance Director has been. directed to initiate a training pro-gram for those personnel involved in quality activities. This training will be coordinated witit the corrective steps taken.
.in response to RO Inspection Report No. 74-09.
The corrective steps taken on the last item which pertains to the inspectors not being independent of any responsibility for performing the work or action he will be inspecting are as follows:
1.
Effective immediately the personnel assigned the inspection responsibility will be independent of the responsibility for performing the work.
2.
To avoid further violations in this area, the Quality Assur-ance Director has been directed to initiate a training pro-gram for those personnel involved in quality activities related to quality assurance activities. This training will be coordinated with item (2) of this report plus the correc-tive steps taken in response to RO Inspection Report No. 74-09.
The following action is being initiated immediately to improve the effectiveness of the overall Rancho Seco Unit No. 1 Quality Assurance Pro-gram:
1.
A non-SMUD employee who was formerly stationed at the Rancho Seco site will be employed t,o temporarily increase the fre-quency of audits.
2.
As noted in the response to the' specific deficiencies, the Quality Assurance Director will initiate additional quality assurance training for those Rancho Seco No.1 personnel involved in quality activities.
3.
A concentrated effort will be made to revise and upgrade the quality assurance procedures that have proven to have poor definition or inadequate detail.
4.
The overall Rancho Seco Unit No. 1 quality assurance audit schedule will be revised and upgraded to give better audit coverage. These audits will be employed to inform management of quality assurance problems in a timely manner.
Should any of-the corrective steps listed not be satisfactory, or if additional information is required, do not hesitate to call.
