s FEB 6 1973 Sacramento Municipal Utility District Docket No. 050-0312 Certain activities under your construction permit appear to be in noncompliance with your quality assurance program requirements as indicated below:
1.
Quality Assurance Procedure No. 9 states (1) "All Special Process -
Procedures shall be approved by the Quality Assurance Engineer," and (2) " contractors submit procedures for special processes they perform for Bechtel (Vernon) Project Engineering approval before use."
Contrary to the above requiremenen radiographic examination of primary coolant pressure boundary pipe welding was conducted using procedures that had not been approved for this project by Bechtel Quality Assurance and Engineering.

Quality Control Instruction No. 301-B requires that, " Low hydrogen or stainless steel type electrodes shall be issued in or placed in portable rod warmers which keep the electroden warm and dry until they are used or returned to the rod room attendent."
Contrary to the above requirement, on December 21, 1972 at approximately 10:00 a.m. and again at noon, portable electrode warmer No. 99 containing cold E7010 electrodes was observed to be unplugged in a work area.
3.
Section 7A.03 " Governing Codes and Scandards" of the M1.1 contract specified the use of the latest applicable issue of the American Welding Society Standards.
Section 408 of AWS Dl.0-69, Welding in Building Constructica, requires low hydrogen E90 series welding electrodes to be stored in ovens held at. 250*F prior to use, and must be redried if not used within one hour.
Contrary to the above requirement for the installation of the steam i
generator lateral support structurca (T-1 steel, E9018 electrodes)
(1) the portable electrode warmers were open with the ends of the E9018 electrodes exposed to the atmosphere Cor periods exceeding one hour and (2) several E90 electrodes were observed to be out of a portable warmer (NE leg of the south steam generator support structure) during the lunch break on December 21, 1972.

N Sacramento Municipal Utility District FEB 6 1973 It is our understanding with respect to the following items that you intend to take the actions indicated:
1.
Construction practicca relating to the usage of portable electrode warmer containers will be examined and corrective measures instituted to comply with the applicable codes.
2.
The entire program for equipment henter control will be examined and measures will be established to cacurc that equipment heators are on.
.3.
The radiographs of the high preocuce injection system makeup pump will be examined to determine the acceptability of the auction nozzle welding.
4.
Additional information will be obtained to t ow that the pressuriner safety and electromatic relief valves were tested at the rated prenourcs.
5.
The records relating to the radiographic examination of pipe welding will be examined and measures taken to permit independent film interpre '
tation.
We will examine your actions on these items at the time of our next inspection.
