4. Occurrence Date: 7 March 1977
5. Identification of occurrence:

e Failure to have two Emergency Ndwater Pumps available and operable from two independent power sources as defined by Technical Specification 3.7.1.2.b.

l 6. Conditions Prior to Occurrence: ,

! At steady state condition in Mode 1, 40% power, a start of 3B Turbine driven . :

Emergency Feedwater Pump was attempted to inspect for packing leakage.

7. Description of Occurrence:

At 1720 on 7 March 1977, 3B Turbine driven Emergency Feedwater Pump was scatted from the Control Board with Main Steam as the supply. The pump cripped on overspeed and was declared inoperable per Technical Specifica- 1

tion 3.7.1.2.b. Further investigation revealed that the pump operated j normally when started locally on Main Steam. The steam source was changed -

to Auxiliary Steam and the pump operated normally both locally and from the l Control Board. With the lineup on Auxiliary Steam the pump was declared j operable. l l

4 8. Designation of Apparent Cause of Occurrence:

j On initial steam admission, Turbine throttle valve opens fully. Steata sup-  !

! ply valve opens sufficiently fast on remote start to admit Main Steam faster  !

9. Analysis of Occurrence:

Redundancy is provided by motor driven Emergency Feedwater Pump which was de-termined available and operable. Turbine driven ENP is operable both lo-i cally and remotely when lined up on Auxiliary Steam. Should this supply '

i become unavailable, startup of the pump can be accomplished manually using Main Steam lineup within the time frame required by the Safety Analysis.

There were no adverse safety consequences resulting from this occurrence.

., 10. Corrective Action:

As a result of this occurrence, Auxiliary Steam lineup is maintained for standby EWP condition. Engineering has been requested to provide means to prevent remote, Main Steam startup problem.

, 11. Failure Data:

There have been no previous occurrences of this type.
