m, Supplementary Information 1.
Report No:
50-302/77-25 2.
Facility:
Crystal River Unit #3 3.
Report Date:
30 March 1977 4.
Occurrence Date:
7 March 1977 5.
Identification of Occurrence:
High pressure injection capabilities to "B" HPI loop inoperable con-t trary to Technical Specification 3.5.2.
6.
Conditions Prior to Occurrence:
Plant was operating normally in Mode 1 at 40% power steady state.
7.
Description of Occurrence:
In order to perform maintenance on one of two "B loop high pressure injection isolation valves (MUV-26), the "B" loop was isolated. During this time period, two independent High Pressure Injection Pumps were available and operable from independent power supplies to the "A" high pressure injection loop. Following maintenance, the "B" HPI loop was restored to original configuration.
8.
Designation of Apparent Cause of Occurrence:
The cause of this event was performance of necessary planned maintenance.
9.
Analysis of Occurrence:
There were no safety implications as either subsystem in conjunction with core flooding would maintain temperatures within prescribed limits in the 4
event of accident conditions and redundancy was maintained.
10.
Corrective Action:
No corrective action is required as Makeup Valve MUV-26 was rapacked and the "B" HPI loop was restored operable within 72 hours8.333333e-4 days
0.02 hours
1.190476e-4 weeks
2.7396e-5 months
in accordance with the Technical Specification.
11.
Failure Data:
This occurrence was non-repetitive.
