Identification of Occurrence:

Inoperability of one pair of Auxiliary Building erhaust fans contrary to Technical Specification 3.7.8.1.

Conditions Prior to Occurrence:

Mode 1 power operation 7.

Description of occurrence:

s At 2120 the Auxiliary Building exhaust fans AHF 14A and AHF 14C were removed from service due to a duct leak located between the fans and the discharge damper. Entered a seven day Action Statement and the duct was repaired with a laminated patch. The system was restored operable on 21 June 1977.

Designation of Apparent Cause:

The cause of this event was determined to be vibration of the system ductwork causing ' a hole between the fans and the discharge damper.

9.

Analysis of Occurrence:

There was no safety significance in that redundant buiiding exhaust system was available and operable.

10.

Corrective Action:

A laminated patch was installed and is expected to strengthen the ductwork to prevent reoccurrence.

11.

Failure Data:

This was the first occurrence of such an event.
