5. Identification of Occurrence:

Unplanned release of radioactive waste gas contrary to Environ-mental Technical Specifications 5.6.2.A.

6. Conditions Prior to Occurrence:

d Normal Mode 1 Operation.

7. Description of Occurrence:

At 1400. in the progress of venting the makeup tank to the waste gas low pressure header, an unplanned radioactive gas '

release to the Auxiliary Building occurred. The maximum instantaneous release rate at the peak of the discharge pro-file was 6.4% of the most restrictive limit set forth in Specification 2.4.2A. (1) . Supply air to the Auxiliary Building was re-established and ventilation of the building resumed s

) within three minutes.

8. Designation of Apparent Cause:

Blown loop seal of waste gas decay tank connected to waste gas low pressure header.

9. Analysis of Occurrence:

The health sud safety of the plant and general public were

. 10. Corrective Action:

The loop seal was re-established and surveillance of loop seals has been increased to preclude re-occurrence.

11. Failure Data:

This was the first reportable occurrence of this type.
