The following is offered as a response to NRC Inspection Report Numbers 50-321/80-4 and 50-366/80-4 concerning one infraction:

As required by Technical Specification 4.5.E.2, the High Pressure Coolant Injection System (HPCI) is to be proven operable immediately when the Reactor Core Isolation Coolant System (RCIC) is found to be inoperable. Technical Specification 3.5.E.3 requires that the reactor be shutdown and depressurized within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> if HPCI is not proven operable.

Contrary to the above, and as reported by the licensee, HPCI was not proven operable for approximately 25 hours2.893519e-4 days <br />0.00694 hours <br />4.133598e-5 weeks <br />9.5125e-6 months <br /> following a RCIC system failure on December 8, 1979. Plant operation continued throughout this interval. The failure to take required actions was due to personnel error. The Shift Foreman, Plant Operator and two Assistant Plant Opera-tors on shift were relieved of licensed duties to complete a formal training and self-study program on technical specifications and proce-dural requirements. Two of these men completed their training and passed an exam on December 17, 1979. The other two completed their training and passed an exam on January 4, 1980. Also, the Superinten-dent of Operations talked with the licensed operators and stressed the

importance of adhering to procedural requirements and complying with technical specifications.
