During Extended Construction Delay

1. OVERVIEW 1.1 This Plan is intended to provide specialized treatment, where appropriate, of Quality Assurance activities during the extended construction delay period at Braidwood Station.

This Plan is a part of Commonwealth Edison's Corporate >

Quality Assurance Program; the policies, requirements, i

and procedures therein remain in effect and unmodified except as specifically identified in this Plan.

1.2 This Plan shall be valid for a period of approximately  ;

one year from the day of substantial cessation of construc-tion. Thereafter, if extended construction delay continues, l this Plan, including any procedures and instructions spec-ific to this Plan, shall be reviewed on an approximately annual basis to insure that measures for identification, preservation, maintenance, and security of systems, compon-ents, materials, and records are adequate for the then-anticipated duration of the delay.

1.3 The objectives of this Plan shall be the maintenance of the identity, integrity, and quality of those safety-related structures, systems, components, equipment, mater-ials, and records on-site as of the date of construction cessation; and the safeguarding of the identity, integrity, and quality of safety-related components, equipment, mater- .

ials, and records which arrive on-site during the construc-tion delay.

1.4 The essentials of this Plan are summarized as follows:

.* 1.4.2 Storage and protection of such identified safety- l related systems, components, equipment, materials, and records in appropriate fashion to prevent the degradation of quality.

1.4.3 Review of incoming items and documents on a timely basis, and Lacorporation of safety-related items and documents under the programs of .1 and .2 above.

1.4.4 Inspection, maintenance, and surveillance of the items and records protected under .2 above, with frequency sufficient to insure that protection and identification measures remain in place and effec-tive.

Pago 3 of 15 II. ORGANIZATION 2.1 The site organizations for the purposes of this Plan remain as described in Commonwealth Edison's Corporate Quality Assurance Manual, Q.R. 1.0.

f 2.2 Because of the limited activities at the site, the person-nel complement on-site from each department involved in quality-related activities may vary.

2.2.1 Site Quality Assurance activities will be under the direction of a Site Quality Assurance Coordi-nator. He will ordinarily be located at the site, but may be absent from time to time for other act-ivities, training, or off-site conferences. When the Q.A. Coordinator is to be unavailable for an extended period (e.g., vacation), other personnel will be made designated to serve in his stead.

5 2.2.1.1 Additional staff, including appropriately qualified Q.A. Engineers or Inspectors, may be assigned to assist the Q.A. Coord-inator on a regular or temporary basis.

i Further the Q.A. Coordinator may request such assistance through the Director of Quality Assurance (Engineering and Const- i ruction) or if not available', of the Man-ager of Quality Assurance.

2.2.1.2 The Q.A. Coordinator will have responsi-bilities as discussed in 1.4.5.1.C of Q.R.

1.0. In addition, he will be responsible for carrying out those Quality Assurance duties covered by this Plan including

i 2.2.2 Site Station Construction activities will be under l

the direction of the Station Project Superintendent I l

or Project Engineer.

2.2.2.1 The Station Project Superintendent or Project Engineer may have staff of crafts-men to assist him as well as qualified field engineers.

2.2.2.2 The responsibilities of the Station Pro-dect Superintendent or Pioject Engineer will be as discussed in 1.4.3.1 and 1.4.3.2 of Q.R. 1.0. In addition, he will te  ;

responsible for carrying out those duties delegated by this Plan to the Station Con-l l

, 4.1 A Documentation Control System will be developed and used to assure that: ,

4.1.1 Contractors' Permanent Documentation on file at the time of work cessation is inventoried and listed and audited.

4.1.2 controlled access to the Contractors' file / vault is maintained.

4.1 3 Incoming Documentation is controlled. All aspects of the Document Control System will be handled in ,

accordance with site procedures, instructions and

,the CECO Q.A. Mnnual.

4.2 The Station Construction Department shall require Contrac- i tors doing safety-related work to provide an inventory of I all permanent records on file at the time of work cessation.

The Quality Assurance Department will then audit these files l

to assure that the Daventory and records are complete, accur-ate and controlled. After such assurance, the Contractor's files / vault will be locked, sealed and controlled access maintained by the Quality Assurance Department.

l' 4.3 During the extended construction delay, incoming Contractors' documentation will be opened, sorted, retained emd controlled by the Station Construction Department. The Document Inven-tory List will be kept current by the Station Construction Department. Quality related documentation to be retained in the Quality Assurance Department vault will be routed to the Quality Assurance Department for filing. CECO material receiving documentation will bc controlled in accord-i ance with the CECO Q.A. Program

i 5.1 Station Construction shall be responsible for establishing an inventory of those safety-related materials, components, l

structures and systems to be protected and/or safeguarded.

5.1.1 Station Construction shall be responsible for up- l l

dating or revisions of such inventory, as required, in accordance with appropriate procedures.

t 5.1. 2 The inventory shall include locations or cross-refer-ences to permit locating each item listed.

5.1. 3 The inventory shall include, or shall be cross-refer-  !

enced to permit identification of, the protection, j preservation, maintenance, and/or safeguarding requirements established for each item under Section VII of this Plan.

5.1.4 Copies of the inventory and of each approved revision l thereof shall be distributed by Station Construction li in accordance with appropriate procedures. l 5.2 Station Construction shall be responsible for placing and maintaining clear and legible identifying markings on each protected item or, in the case of bulk materials, on each lot.

5.3 Station Construction shall be responsible for control of the activity, including maintenance, assembly, diasssembly, fur-ther construction, relocation, or operation, upon any pro-tected structure, system, component, equipment, or material; and for documenting any such activity and updating any docu-ments affected thereby.

l Page 9 of 15 VI. CONTROL OF TEST EQUIPMENT

. 6.1 All test equipment shall be controlled in accordance with the CECO Q.A. Program, Q.R. 12.0 and related Quality Pro-cedures. This will include calibration status and ident-ification traceable to calibration standards.

6.2 Identification tags or stickers shall indicate the calibra-tion status and requirements.

' Page 10 of 15 VII. HANDLING AND STURAGE 7.1 Station Construction Department will be responsible for receiving and processing incoming shipments in accordance with Q.P. 7-1.

'., 7.1.1 The Site Quality Assurance Coordinator will be re-sponsible for those duties specified in Section 4.3 of Q.P. 7-1.

7.1.2 Station Construction shall classify all safety-related items received as to any requirements for

preservation, protection, maintenance, and/or

, safeguarding, and shall add any items having such requirements to the inventory required under the.

provisions of Section V, above.

7.2 Station construction Department will provide for effective preservation, protection, maintenance, and/or safeguarding of all safety-related materials, components, systems, and structures identified as requiring such protection, preser-vation, maintenance, and/or safeguarding under the provisions of Section V, above.

7.2.1 Each item or class of safety-related equipment, structures, or systems will be reviewed to deter-mine what, if any, protection, preservation, main-tenance, and/or safeguarding is requir'ed to prevent significant deterioration of quality during the ex-tended construction delay. 7.2.1.1 Such review will consider, but not be lim-ited to, the following: Required environ- ,

7.3 Station construction shall establish procedures for mainten-

ance, servicing, operation or exercising at the specified intervals, and shall perform or arrange for such activity. >

7.4 Station Construction shall establish procedures for inspec--

tions, surveillances, and/or testing of all inventoried items at frequencies sufficient to insure that all protec-tive requirements are being met.

Page 13 of 15 VIII. NONCONFORMING ACTIONS / ITEMS AND CORRECTIVE ACTION 8.1 All items found to be nonconforming during the duration of the extended shutdown will be controlled and processed -

in accordance with the CECO Q.A. Program, Q.R.15.0 and ,

related Quality Procedures. Hold areas and Hold tags will ,

be provided and utilized. Nonconformances will be written and proper dispositions and follow-up action to prevent any recurrence of such nonconformances will be required.  !

Station Construction will be responsible for initiating, i processing and closing out Nonconformances.

, IX. AUDITS 9.1 During the extended construction delay there will be scheduled audits in accordance with CECO's O.A. Program Q.R.18.0 and related Quality Procedures. The Q.A. Site  !

Coordinator will establish an approved audit schedule to assure that procedures and duties cited in this Plan are carried out. All Audits will be performed to an approved [

checklist by qualified auditors.

9.2 Follow-up surveillances and audits to detect recurrence of any deficiencies, or to close out audits, will be in accordance with the CECO Q.A. Program.  :

X. ACCESS CONTROL i

10.1 Station Construction Department shall be responsible for providing and maintaining barriers surrounding protected areas of the site;

, areas in which safety-related systems, components, equipment, structures, or materials are loca-ted shall be within one or more such protected areas.

10.2 Station Construction shall be responsible for providing security systems and/or. personnel to control access to protected areas.

10.3 Station Construction shall be responsible for controlling distribution of keys or other access devices, where such devices are required to control access to areas containing protected material, structures, components, equipment, or systems. i R. Cosaro

', Extended Construction Delav at Braidwood Station 1-1.0 PURPOSE:

1-1.1 The purpose of this procedure is to assure that all safety-related documents are adequately controlled to prevent loss or damage throughout the csnstruction delay period.

1-2.0 SCOPE:

1-2.1 This procedure applies to all safety-related con-tractor documents excluding construction drawings.

1-3.0 PROCEDURE:

1-3.1 A documented and detailed inventory listing every safety-related document in the contractor's posession at the time safety-related work was halted shall be  :

prepared by the contractor. This inventory shall be reviewed and accepted by CECO Q.A..

1-3.2 Using the document inventory, an audit of the con-tractors' files will be conducted by CECO Q.A. to assure that the documents listed in the inventory are being maintained in the appropriate files. All .

discrepancies must be resolved at this time.

1-3.3 After the audit is completed and the discrepancies  !

are resolved CECO Q.A. shall lock and seal the file cabinets / vaults.

1-3.4 Access to the files will be limited and controlled by CECO Q.A.. Anyone wanting to enter a file for any reason must contact CECO Q.A. so that the seals can be broken and the file unlocked. If a document is ,

to be removed from a file the document control log (See Attachment A) must be filled out by CECO Q.A.

and the files relocked and resealed.

1-3.5 When a document is returned to the file CECO Q.A.

shall complete the document control log, the seals will be broken, the cabinet / vault vill be unlocked, the documents filed, and the file cabinet / vault will be relocked and resealed.
