eo APPENDIX A NOTICE OF VIOLATION Consumers Power Company Docket No. 50-329 Docket No. 50-330 Based on the inspection conducted on January 8-11, 1980, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below. Item 1 is an infraction, item 2 is a deficiency.

10 CFR 50, Appendix B, Criterion IX, states, in part, that, " Measures shall be established to assure that special processes, including
- welding,
, are controlled and accomplished by qualified personnel.
Consumers Power Company Quality Assurance Program Policy No. 9, Revision 7, states, in part, "For such special process as welding,
, Consumers Power, its suppliers, and their lower tier suppliers, accomplish these processes under controlled conditions in accordance with applicable codes, standards, and specifications using qualified procedures, equipment, or personnel..."
Babcock & Wilcox Construction Company procedure No. 9-WG-100, paragraph 1, states, in part, that "This procedure shall govern the performance qualification cf welders using the Shielded Metal i
Arc (SMAW) and Gas Tungsten Arc (CTAW) welding processes in accor-dance with Section IX of the ASME Boiler and Pressure Vessel Code."
ASME B&PV Code Section IX limits a welder to weld on twice the thickness of the test plate when the test plate is less than 3/4" thick.
Contrary to the above, on January 10, 1980, the inspector established that welder I.D. No. 624 was qualified to weld on maximum thickness of 0.686".
Records reviewed by the inspector indicated that this welder welded on Unit 1 Reactor Coolant System, Weld No. WJ1-1 between August 13 to 29, 1979. The minimum wall thickness of this weld joint is 2 5/8".

Consumers Power Company Quality Assurance Program No. 17, Revision 7, states, in part, tha t, ".
. Inspection and test records as a minimum identify the inspector or data recorder, the type of observation, the results, the acceptability, Bechtel Power Corporation Procedure No. SF/ PSP-G-6.1, Revision 4, Paragraph 4.3.g requires that the date the inspection activities were completed be recorded on the Quality Control Inspection Record.
Contrary to the above, on January 10, 1980, the inspector established that the cleanliness inspection of Unit 2 Service Water System valve Serial No. D0061-1-3 was not dated or signed off on the Quality Control Record.
