9 APPENDIX A NOTICE OF VIOLATION Virginia Electric and Power Company License No. DPR-32 Surry 1 Based on the NRC inspection November 1 - December 14, 1979, certain of your activities were apparently not conducted in full compliance with NRC require-ments as indicated below. These items have been categorized'as described in correspondence to you dated December 31, 1974.
As required by_ Technical Specification 6.4.A.7 and 6.4.D, the detailed written procedures provided for preventive or corrective maintenance optrations which would have an effect on the safety of the reactor must be
-followed.
Contrary to the above, on November 20, 1979, the inspector observed that the heat-traced Unit 1 emergency borate line (2" CH-42-152) was not properly insulated, indicating that corrective maintenance procedures EMP-C-HT-20 or MMP-C-G-017, which require reinsta11ation of the insulation removed, were not followed. The missing insulation may have led to or aggravated the plugged condition of the borate line.
This is an Infraction.
