- m y.~d APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority License No. CPPR-72 Sequoyah, Unit 1 Based on the NRC inspection November 28 through December 1, 1979 and December 3-7, 1979, certain of your activities were apparently not conducted in full compliance described in correspondence to you dated December 31, 1974.
As required by 10 CFR 50, Appendix B, Criterion V, activities affecting quality shall be accomplished in accordance with documented instructions.
The licensee's accepted QA program, FSAR, Section 17.1A.5, states that activities affecting quality are prescribed by documented instructions in the form of drawings, specifications and procedures. Administrative In.
struction A.I.9, Temporary Alterations, which provides instructions for implementing this requirement, states that alterations will be properly documented and approved.
Contrary to the above, on December 5,1979, temporary alterations in the form of jumpers were found in panel R17, on terminal board 17B, and in panel R14, on terminal board 14B. These jumpers met none of the approval and documentation requirements of Administrative Instruction AI-9.
This is an infraction.
