APPENDIX A Sacramento Municipal Utility District P. O. Box 15830 Sacramento, California 95813 Docket flo. 50-312 License No. DPR-54 Notice of Violation Based on the results of the NRC inspection conducted on November 30,1979, it appears that two of your activities were not in full compliance with applicable NRC regulations and conditions of your license, as indicated below.

A.

Technical Specification 6.9.4.1.b requires a report to be submitted within twenty-four (24) hours by telephone and confirmed by telegraph, mailgram or facsimile transmission to the Director of the Regional Office if opera-tion of the unit is less conservative than the least conservative aspect of the limiting condition for operation.

Contrary to the above, documentation between J. J. Mattimoe, Assistant General Manager and Chief Engineer (SMUD), and W. Gammill, Acting Assistant Director for Operating Reactor Projects (DOR-NRC), dated October 19, 1979, indicates knowledge of a degraded voltage condition, but this condition was not reported to Region V until November 1,1979, via the Resident Inspector.

This item is a deficiency.

B.

Technical Specification Table 2.3-1 specifies that the Nuclear Power trip setting is 5% of rated maximum when the plant is in the shutdown bypass mode.

Contrary to the above, at 0635 on November 25, 1979, the plant was placed in the shutdown bypass mode, but the Nuclear Power trip was not reset to 5% of rated maximum.

This item is an infraction.
