Dear f1r. Engelken:

We have reviewed the notice of violatior that two of our acti-vities were not in full compliance with the condition of our license:

A.

Technical Specification 6.9.4.1.b requires a report to be submitted within twenty four (24) hours by telephone and confirmed by telegraph, mailgram or facsimile transmission to the Director of the ~.egional Office if operation of the unit is less conservative than the least conservative aspect of the limiting condition for operation.

We acknowledge that Regulatory Guide 1.16, Reporting of Operating Information, Appendix A, Technical Specifications and the Technical Speci-fications Section 6.9.4', Reportable Occurrences, requires twenty four (24) hour reporting requirements and confirmation by telegraph, mailgram or facsimile transmission to the NRC Regional Office within two weeks. When the events connected with the reportable occurrence are confined to the Rancho Seco site this requirement is attainable and routinely accomplished.

If the information occurs external to the Rancho Seco site, the definitive information regarding licensee report events (a) through (i) of Section 6.9.4.1 is not always apparent tecause correspondence from vendors and contractors may or may not be direded to proper District personnel.

Therefore, we must define the twent" four (24) hour requirement to be identified from the time this infc.wation is obtained by the cognizant engineer or his supervision.

A training program will be initiated by Ouality Assurance / Nuclear Operations to acquaint the engineering staff of Generation Engineering of the reporting requirements, the contents of Regulatory Guide 1.16 and Technical Specifications Section 6.9.4.

4 Mr. R. H. Engelken Janu'ry 28, 1980 This training program will be conducted annually and will enlist the aid of the Chairman of the PRC to answer any questions connected with evaluations of items falling under the requirements.

The initial train-ing sessions will be started by 1 March 1980 with training of all personnel completed by 1 July 1980.

Technical Specification Table 2.3-1 specifies that the Nuclear Power trip setting is 5% of rated maximum when the plant is in the shutdown bypass mode.

Contrary to the above, at 0635 on November 25, 1979, the plant was placed in the shutdown bypass mode, but the Nuclear Power trip was not reset to 5% of rated naximum.

A Special Order 16-79 was issued to all operating personnel on December 4, 1979. The Order reaffirmed that in accordance with our plant cooldown procedure, B.4, when placing the RPS in shutdown bypass, the high flux trips are to be reset to <S%.

If plant conditions warrant this position to be carried out later, a temporary change will be written to the procedure and noted in the log book.

This notification and the required do mmentation will prevent the Nuclear Power trip setting from being missed during the period when the RPS system is put into shutdown bypass.
