Road Mo nstown, New Je sey 07960 (20" ~55-8200 OYSTER CREEK NUCLEAR GENERATING STATION Forked River, New Jersey 08731 Licer.see Event Report Reportable Occurrence No. 50-219/80-8/IT Recort Date February 29, 1980 Occurrence Date February 15, 1980 identification of Occurrence During verification of as-built conditions for piping hangers, it was noticed that nine (9) pipe clamps on piping to the isolation condensers were not pro-perly installed.

This event is considered to be a reportable occurrence as defined in the Techni-cal Specifications, paragraph 6.9.2.a. (9).

Conditions Prior to Occurrence The plant was shutdown for a refueling / maintenance outage.

The reactor was subcritical.

The reactor mode switch was locked in refuel.

The cavity was flooded.

Description of Occurrence On Friday, February 15, 1980, during the inspection required for IE Bulletin 79-14, nine pipe clamps were discovered improperly installed.

The clamps which connect the snubbers to the isolation condenser piping were not restrained by welded lugs.

The snubbers, as referenced by Bergen-Tatterson Drawing, are numbered:

Analvsis of Occurrence The snubbers are attached to the pipe clamps in three different methods.

One method is connecting the snubber line of direction through the center line of the pipe.

Inis method does not require welding of clamp to the pipe because no sliding can occur.

The other methods of attachment are axial and torsional.

These two methods require welding of the clamp to the pipe because there would be a ;0ssibility of the clamp sliding during a seismic event.

The pu pose of tne Isolation Condenser System is to depressurire the reactor anc to rencve reactor decay heat in the event that the turbine generator and main conde'ser are unavailable as a heat sink.

The system consists of two loops, each ca:able of accomplishing the purpose of the svstem.

If a seismic event would have occurred, the pipe restraint may not have functiored as recuired.

Corrective Action

The nine clamps will be prevented from sliding during a seismic event by attach-ing welded lugs to the piping.

The lug placement has been determined by a Burns G Roe review and approved by Generation Engineering.
