Gentlemen:

This refers to the special inspection conducted by Messrs. L. J. Hueter and

N. J. Chrissotimos of this office on December 11-14, 1979, of activities at

the Quad-Cities Nuclear Power Station, Unit 2, authorized by NRC Operating

License No. DPR-30 and to the discussion of our findings with

Mr. N. J. Kalivianakis at the conclusion of the inspection.

Additionally,

the results of this inspection and cur related concerns were discussed dur-

ing a meeting held at your office in Chicago, Illinois on January 10, 1980,

which was attended by me and others of my staff and Mr. C. Reed and others

of his staff.

The enclosed copy of our inspection report identifies s eas examined during

the inspection. Within these areas, the inspection consisted of a selective

examination of procedures and representative records, observations, and in-

terviews with personnel.

During this inspection, certain of your activities appeared to be in noncom-

pliance with NRC requirements, as described in the enclosed Appendix A.

Also in connection with the unplanned release of radioactive liquid effluent,

we are concerned with the apparent inattention to duties exhibited by two

licensed senior operators (shift foreman) in directing the activities of non-

licensed operators on their shift during the draining evolution involving

the Unit 2 RHR heat exchanger which lead directly to this event.

IE Infor-

mation Notice No. 79-20, Rev. 1 issued on September 7, 1979, noted that the

NRC would more closely monitor the performance of NRC licensed individuals,

and would take appropriate enforcement action against such individuals for

significant violations of their license.

After reviewing this event and

evaluating its relative safety significance we have decided that no en-

forcement action will be taken against the individual senior operators.

However, we wish to point out that, while we expect licensee management

to assure that adequate procedures are in place and that senior operators

receive adequate training, we also expect licensed senior operators to

assess the safety significance of their actions. We plan to review the

actions of the Quad-Cities licensed senior operators during future in-

spections.

This notice is sent to you pursuant to the provisions of Section 2.201 of

the NRC's " Rules of Practice," Part 2, Title 10, Code of Federal Regulations.

Section 2.201 requires you to submit to this office within twenty days of your

receipt of this notice a written statement or explanation in reply, including

for each item of noncompliance: (1) corrective action taken and the results

achieved; (2) corrective action to be taken to avoid further noncompliance;

and (3) the date when full compliance will be achieved.

In accordance with Section 2.790 of t.he NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and the enclosed

inspection report will be placed in the NRC's Public Document Room, except as

follows.

If this report contains information that you or your contractors

Lelieve to be proprietary, you must apply in writing to this office, within

twenty days of your receipt of this letter, to withhold such information from

public disclosure. The application must include a full statement of the rea-

sons for which the information is considered proprietary, and should be pre-

pared so that proprietary information identified in the application is con-

tained in an enclosure to the application.

We will gladly discuss any questions you have concerning this inspection.
