Gentlemen:

This refers to the inspection conducted by M. S. Kidd of this office on April 14 -

Mav 30, 1980, of activities authorized by NRC Li cense Nos. NPF-4 and NPF-7 for

the North Anna facility, and to the discussion of e r findings hAd with

W. R.

'artwright at the conclusion of the inspection.

Areas examined during the inspection and our findings are discussed in the

enclosed inspection report.

Within these areas, the inspection consisted of

selective examinations of procedures and representative records, interviews with

personnel, and observations by the inspecters.

We have examined actions you have taken with regard to previously identified

enforcement matters and unresolved items. The status of these items is discussed

in the enclosed report.

During the inspection, it was found that certain activities under your license

appear to be in noncompliance with NRC requirements. These items and references

to pertinent requirements are listed in the Notice of Violation enclosed herewith

as Appendix A.

This notice is sent to you pursuant to the provisions of Section 2.201

of the NRC's " Rules of Practice," Part 2, Title 10, Code of' Federal Regulations.

Section 2.201 requires you to submit to this office, within 20 days of your

receipt of this notice, a written statement or explanation in reply including:

(1) corrective steps which have been taken by you and the results achieved; (2)

corrective steps which will be taken to avoid further noncompliance; and (3) the

date when full compliance will be achieved.

During the inspection it was found that certain activities under your license

appear to deviate from commitments to the Commission and have safety significance.

These items identified in the Notice of Deviation enclosed herewith as Appendix B.

Please provide us in writing within 20 days of your receipt of this letter your

comments including a description of corrective actions that have been or will be

taken, corrective actions which will be taken to avoid further deviations, and

the date corrective actions were or will be completed.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2,

Title 10, Code of Federal Regulations, a copy of this letter and the enclosed

insp ction report will be placed in the NRC's Public Document Room.

If this

report contains any information that you (or your contractor) believe to be

proprietary, it is necessary that you make a written application within 20 days

to this office to withhold such information from public disclosure.

Any such

application must include a full statement of the reasons on the basis of which

it is claimed that the information is proprietary, and should be prepared so

that proprietary information identified in the application is contained in a

the specified period, the report will be placed in the Publit Document Room.

Should you have any questions concerning this letter, we will be glad to discuss

NOTICE OF VIOLATION

Based on the NRC inspection April 14 - May 30, 1980, certain of your activities

were apparentl'. not conducted in full compliance with h7C requirements as indicated

below. These items have been categorized as described in correspondence to you

dated December 31, 1974.

A.

As required by Unit 1 Technical Specification 6.8.lc, written procedures

shall be established, implemented and maintained for conduct of surveillance

and test activities of safety related equipment.

Contrary to the above, Unit 1 periodic test procedure 1-PT-36.10, "P-4

Permissive Verification," written to verify proper operation of reactor

trip breaker auiliary contacts feeding P-4 every 31 days, was not performed

between the dates of January 14, 1980 and May 8,1980.

This is an Inf raction.

B.

As required by Criterion V of Appendix B of 10 CFR 50 as implemented by

Paragraph 17.2.5 of VEPCO's " Topical Report - Quality Assurance Program -

Operations Phase" (VrP-3A) requires that activities affecting quality be

p rform-d in accordance with procedures.

Contrary to the above, rework of the Unit 2 control room air conditioning

chillers and their control panels per Engineering and Design Coordination

Report P-2717-2 was accomplished without the use of a Rework Control Form.

Use of the rework control form is required by Construction Quality Control

Procedure 26.1 " Rework Control Program."

This is an Infraction.

APPENTIX B

NOTICE OF DEVIATION

Based on the results of the NRC inspection conducted on April 14 - May 30, 1980

certain of your activities appear to deviate from your commitments to the Commission

,

as indicated below.

A.

VEPCO letter Serial No. 1160 dated December 28, 1979, a supplement to

Unit 1 LER 79-149, stated that station emergency procedures would be revised

as

necessary to assure continued ventilation flow from the safeguards

buildings and the auxiliary building central area in the event of ductwork

failure during a seismic event.

Contrary to the above, Units 1 and 2 abnormal Operating Procedure 1-AP-35,

" Seismic Event," was

revised March 5,

1980, but instructions were not

provided for assuring continued ventilation of the safeguards buildings in

the event of failure of exhaust ductwork.

B.

VEPC0 letter Serial No. 123 of February 14, 1980, a final report per 10 CFR 50.55(e)

on the non-seismically qualified control room air conditioning chiller

control panels, stated that required Unit 2 modification would be completed

prior to initial fuel loading.

Contrary to the above, electrical modifications per Engineering and Design

Coordination Report P-2717-2 were not completed until April 28, 1980.

Initial fuel loading began April 12, 1980.

C.

VEPCO's " Topical Report - Quality Assurance Program - Operations Phase"

(VEP-3A), Section 17.2.27,

as implemented by Paragraph 2.5.3.2 of the

" Nuclear Power Station Quality Assurance Manual," requires that temporary

personnel be indoctrinated in VEPCO's quality assurance procedures as

related to their specific tasks to be performed.

Contrary to the above, the Babcock and Wilcox startup test engineer

cha rge of Unit 2 precritical test 2-SW-11 on the 4:00pm and midnight shift

in

on May 1,1980, had not received such indoctrination.

D.

VEPCO letter Seri 1 No. 1179 dated December 12, 1979, concerning commitments

to correct identified equipment problems, stated that any use of service
