Inspection on October 1-2, 1980 (Report No. 50-358/80-22)

Areas Inspected:

Inspection of safety related hangers, restraints, snubbers,

and related QA/QC program provisions. The inspection involved a total of 14

inspector-hours onsite by one h"'C inspector.

Results: Of the areas inspected, two apparent items of deviation were

identified relative to failure of the licensee to implement its response to

noncompli ance items documented in R11I Inspection Zeport No. 50-358/80-05.

Licensee Action on Previously Identified Items

(0 pen) Noncompliance Item (358/80-05-01):

Inadequate document control on

S&L specification and desiga engineering procedures. The inspector reviewed

Supplement 20 to S&L Specification H-2256, " Erection of Phase II Piping

Systems and Mechanica4 Equipment", dated July 14, 1980, and determine)

that (1) DDC's have been incorporated into Supplements 1 through 20, (2)

effective DDC's have been attached in the specification, and (3) specifica-

tion has DDC's listings that are being reviewed. The inspector considers

the corractive actions to be adequate, however, the control of design

engineering procedures will be reviewed at S&L corporate office during a

future inspection.

(Closed) Unresolved Items (358/80-05-02; 358/80-05-03: The lack of es-

tablished personnel qualifications for NPS design staff, verification of

personnel background by the employer, and indoctrination and training

prograa. The inspector reviewed CG&E Audit Report No. 321 of NPS, con-

ducted July 30-31, 1980, subject " Implementation of Design Control Pro-

ced' ires ;

Indoctrination and Training; and Interface with S&L", and CG&E

QASS Surveillance Report No. 277, conducted September 23-26, 1980, that

closed out deficiencies identified in Audit Report No. 321, and had no

adverse comment. The inspector also reviewed S&L External Audit No. E-1

of NPS, conducted April 8-9, 1980, subject " Imp'ementation of QA Program,

Procedures and S&L Specification 101 and 102, Personnel Qualification and

Verification of Worker Education and Work Experience", and S&L External

Audit No. E-1.1, conducted June 24. 1980, that c79 sed out deficiencies

identified in Audit No. E-1, and had no adverse comments.

(Closed) Unresolved Item (358/80 05-04):

Lack of hanger design guidance.

The inspector reviewed the S&L " Mechanical Compoaent Support Steel Design

Procedures" dated March 24, 1980, and the S&L Calculation No. 876, " Mechan-

ical Hanger Design Reference Manual", Revision 0, dated February 28, 1980,

and considers the matter resolved.

(Closed; Noncompliance Item (358/80-13-01):

Lack of design control for the

pipe whip restraints FWFR-1029 and FWFR-1030 connecting boics.

The inspector

discussed the issue with the S&L Senior Structural Engineer, CG&E's QA Engineer,

and HJK's QA Engineer.

DDC's to document the misalignment problems, ASTM

material specifications, and AISC design manuals were presented.

Because of

the explanations, the inspector concurred with the licensee that no apparent

noncompliance exists as stated in the response letter.

NRC internal records

will be changed as appropriate.

Functional or Program Areas Inspected

Fellowup inspcetic7 of site small bore pipe support and restraint design and

installation problems ident ified in RIII Inspection Report 'Jo. 50-358/80-05.

1.

Site L~ sign and Installation of INX (Instrumentation) Pipe Supports

Since September 8, 1980, responsibility for the above subject activities,

was contracted to Nuclear Power Services, Inc. (NPS), and Nuclear Services

Corporation (NSC) personnel at the site. The work is defined in S&L

Consultant Specifications No. 105A (for NPS) and No. 105B (for NSC), "INX

Piping Supports Design and Related Activities", dated July 15, 1980. The

present assignment is that NPS will handle INX supports inside the diesel

generator buildings and the service water pump structure, and NSC will

har.dle supports in the reactor building cutside primary containment and

the auxiliary building. At present, there is a hold on NX support details

in primary containment (CG&E memorandum 80-176, dated Septem er )?, 1980)

to curtail installation of NX support details from h&L drawings M-471,

Sheets 38, 40, 46, and 47 inside primary containment. The NX support

design groups (Specification 105 A and B) will issue designs f;r in-

stallation. HJK was requested not to install NX supports unless they

are issued by the Specification 105 contractor or authorized by CG&E

General Construction Department.

Review of Procedures

-

In conjunction with paragraph 1 of RIII inspection Report 50-358/80-05,

the inspector performed further review of the following HJK field

construction procedures, HJK inspection procedures, and S&L criteria.

FCP 2-6, " Design Document Change", Revision 8, dated August 14,

1980.~ - No adverse comment.

b.

FCP 6-1, " Nonconforming Material Control", Revision 1, dated

February 12, 1980. - No adverse comment.

c.

FCP 2-135, " Process Pipe Support Installation After S&L Final

Design Aanalysis and NX Instrumentation Pipe Support Installation

Criteria", Revision 9, dated July 17, 1980. - No adverse comment.

d.

FCP 2-27, "S&L Review of KEI Drawings", Revision 0, dated

October 16, 1980. - No adverse comments.

S&L EMD-017629 included in Specification H-2256, Supplement 20,

" Criteria for Weight and Seismic Control of 1/2", 3/4", and 1"

Pipe and Tube for Reactor and Containment Buildings", Revision 1,

dated January 2, 1980. - No adverse comments.

FCP 2-115, " Pipe Support Installation - Small Bore Piping",

Kevision 1, dated December 7, 1977, which states in Paragraph

3.3.1, " Construction Inspection Plans (CIP) shall be used to

acceptability of essential piping only"

The inspector questioned

what was a CIP and what was the control procedure in applying

the CIP.

Subsequently, Revision 2 of the procedure, dated

October 1, 1980, paragraph 3.1.0 reads, " Construction inspection

shall be in accordance with QACMI M-12 for essential piping

support only".

FCP 2-134, " Installation of NX Type Pipe 9upports", Revision 1,

datod March 16, 1979, which states in paragraph 3.2.1, "Const ruc-

tion inspection shall be in accordant e with QACMI M-16 for

seismically supported pipe only".

QACMI M-lo was replaced by

QACMI M-12 on June 23, 1980, and this was not reflected in FCP

2-134.

Subsequently, Revision 2 of the procedure, dated

October 2,1980, revised paragraph 3.2.1 to read " Construction

inspection shall be in accordance with QACMI M-12 for seismically

supported pipe only".

h.

QACMI M-12, " Inspection Instructions for Pipe Hangers, Support

and Restraint Installation", Revision 10, dated September 16,

1980. The inspector commented that timely incpection of tastalled

hangers was not required specifically in the procedure in two

categories:

Installation of hangers for S&L final approved drawings

Installation of hangers per site DDC's permitted by FCP

2-6, paragraph 3.2.2, which states,, in part, that "When the

DDC receives preliminary approval the work can proceed and

the formal S&L approval to follow through regular channels"

' Subsequently, Revision 11, dated October 2, 1980, was issued

and included the following requirements:

Paragraph 3.2:

"The hanger engineer shall submit the support package to

the inspection supervisor for review, who in turn shall

transmit the package to the lead inspector. The lead

inspector shall provide for inspection of +.he support

within thirty (30) days of receipt of the support pa"kage"

" Inspection personnel shall verify that the latest appropri-

ately approved drawings and applicable DDC's have been

utilized for the installation of essential and nonessential

seismic pipe hangers, supports, or restraints"

Exit Interview

The inspector met with the licensee representatives (denoted under Persons

Cuntacted) at the conclusion of the inspectiva on October 2, 1980. The

inspector summarized the purpose and findings of the inspection.

The

licensee acknowledged the findings reported herein.
