Appendix A NOTICE OF DEVIATION Cincinnati Gas and Electric Company Docket No. 50-358 Based on the inspection conducted on October 1-2, 1980, it appears that certain of your activities were not in ar ordance with commitments, as noted below.
Your response dated July 8, 1980, to IE Inspection Report 50-358/80-05 stated that certain actions would be taken to correct your program for the citations found by our inspector and documented in RIII Report No. 50-358/80-05.
Contrary to the above, the following deviations from your response were found:
1.
Contrary to Item 1 of the response, DDC No. M-10744 was not followed up with written request for approval.
During this inspection, your site staff stated that no DDC's will be approved by Stt, instead, a new program that requires S&L to issue each design detail drawing for the small bore process and instrumentation pipe hanger will be in place.
2.
Contrary to Item 3 of the response, S&L did not provide appropriate design guidance on installing shims on excessive restraint gaps.
Clearance requirements on support sleeves shown on S&L drawing M471, Sheet 39 was not clarified. No Nonconformance Report was written to document and resolve the improperly installed hangers. No program was initiated to re-evaluate the past DDC's used to document noncon-formances.
