APPENDIX A fl0TICE OF VIOLATION Vermont Yankee fluclear Power Docket tio. 50-271 Corporation Based on the results of an f1RC inspection conducted on October 22-31, 1979, it appears that one of your activities was not conducted in full compliance with the conditions of your NRC Facility License No. DPR-28, as indicated below.
This item is an Infraction.
Technical Specification 3.5.C.2 states:
"From and after the date that one of the RHR service water pumps is made or found to be inoperable for any reason, reactor operation is permissible only during the succeeding thirty days unless such pump is sooner made operable, provided that during such thirty days all other active components of the RHR service water subsystem are operable."
Technical Specification 4.5.C.2 states:
"When it is determined that one RHR service water pump is inoperable, the remaining components of that subsystem and the other RHR service water subsystem stall be demonstrated to be operable immediately and daily thereafter."
Contrary to the above, on September 14, 1979 at approximately 1530 hours0.0177 days <br />0.425 hours <br />0.00253 weeks <br />5.82165e-4 months <br />, the "A" Residual Heat Removal Service Water (RHRSW) pump was intentionally made inoperable for maintenance without immediately demonstrating the operability of the redundant RWRSW subsystem.
The failure to immediately demonstrate the operability of the redundant RHRSW subsystem was identified by the Shift Super-visor's Log entry which stated:
" Adjustments were made to the mechanism and indication restored to normal before alternate Technical Specification testing could be started."
