Minutes of 197th AC;S theting September 9-11, 1976 D. Millstene-2: In-d Shedding -
D. G. Mcdonald, bTC Staff, identified the sequence of events that occurred at the Millstone facility on July 5 and July 21 during a load shedding operatienp h.e safety significance of these events, and the actions taken by both the licensee and the'NFC Staff.
The events resulted in the following conditions:
o a sustained degraded grid voltage, approx!=ately 93% of
' normal, blem control fuses in the 430 volt motor control conter, erroneous incorrect setting o_f the under voltage trip set point, and,
)
o faulure of the diesels to properly sequence the lead zone (for details of the effects of the conditions, and the rc=edial acticas taken, see Appendix XVIII).- Ee stated t; hat NPC Staff has met with the five operating utilities in order to deter =ine all of the conditions which were involved in this particular problera. The NPC Staff has also issued a detailed request for additional.informatien to aid in the assessment and th* for=ation of a basis for the generic aspects of the pechlem.
