License No. 48-03665-02 Based on the inspection conducted on November 28, 1978, it appears that certain of your activities were in noncompliance with NRC requirements, as noted below.
This item is a deficiency.
10 CFR 34.25(c) requires that records of leak test results be maintained for inspection by the Commission.
Contrary to the above requirement, there was no record available on the date of this inspection of a leak test stated to have been made on your cobalt-60 source between December 5, 1977 and September 1, 1978.
