APPENDIX A NOTICE OF VIOLATION Maine Yankee Atomic Power Company Docket No. 50-309 Based on the results of the NRC inspection conducted on December 18-21, 1978, it appears that one of your activities was not conducted in full compliance with NRC regulations and the conditions of your license as listed below. This item is an infraction.
Technical Specification 5.8 states, in part: " Written procedures shall be established, implemented, and, maintained covering... the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972..." Regulatory Guide 1.33 lists, in part, under Section 1, " Administrative Procedures,... j. Bypass of Safety Functions and Jumper Control." Plant procedure 16-1 " Maine Yankee Operation Safeguard, Yellow Tag - Control Log" paragraph 3.2 states, in part, that Jumper Control Requests "...shall be used for any work requiring blocking or bypassing safety functions involving an operating system or equipment."
Contrary to the above, on four occasions between November 28 and December 20,1978 (as determined from the Control Room Operators' Log and direct cbservation by the inspector), individual RPS channel trip functions were bypassed without completion of a Jumper Control Request.
