Appendix A NOTICE OF VIOLATION Detroit Edison Company Docket No. 50-341 Based on the inspection conducted on January 24-26, 1979, it appears that certain of your activities were in noncompliance with NRC require-ments, as noted below.
These items are infractions.
1.
10 CFR 50, Appendix B, Criterion VII requires that measures be established to assure that purchased material, equipment and services whether purchased directly or through contractors and subcontractors, conform to the procurement documents.
These measures are to include provisions, such as objective evidence of quality furnished by the contractor and examination of the products upon delivery.
Edison QA Manual, QAP No. 7, Paragraph 7.1.4.3 states in part,
" Prior to the release of shipment, QA Level I material and equipment requiring source inspection shall be inspected for conformance to the procurement document requirements."
Contrary to the above, a reinspection of the shopwelds on four beams supplied by Schreiber Manufacturing Company, intended for the Steam Tunnel Lower Pipewhip Restraint Structure, indicated that some areas of the welds did not meet the procurement documents. The reinspection was performed by the licensee after the NRC inspector expressed concern about the welds on two of these beams during a previous inspection.

10 CFR 50, Appendix B, Criterion V requires that activities affecting quality be prescribed by documented instructions, procedures or drawings and be accomplished in accordance with these instructions, procedures, or drawings.
Edison QA Manual, QAP No. 9, Paragraph 9.1.5 states " Activities related to the performance of quality assurance functions affect quality and shall be performed in accordance with prescribed instruc-tions, procedures or drawings.
Contrary to the above, the NRC inspector determined that a Magnetic Particle examination was performed on January 10, 1979, on a weld identified as Field Weld 5B Top even though the surface of the weld had not been cleaned to meet the requirements of Paragraph 9.1 of Magnetic Particle Procedure WB-Q-108 Revision 4.
For example, surface defects such as flux spatter and other irregularitics on the weld were still present on January 25, 1979.
