License No. DPR-50 APPENDIX A NOTICE OF VIOLATION Ba. sed on the results c f the NRC inspection conducted on April 6 and April 10 through April 15, 1978, it appears that one of your activities was not conducted in fu'l compliance with the cenditions of your license as indicated below. T'as item is an Infraction.

Technical Specification 6.8.1 states in part, " Written procedures and administrative policies shall be established, implemented and maintained..."
Contrary to the above, at approximately 2:00 p.m. on Acril 14, 1978, valve SA-V34 was found shut when required by Enclosure III to Surveillance Procedure 1303-6.1, " Reactor Building Integrated Leak Rate Test", Revision 6, dated March 1,1978 with TCN 78-135, dated April 7,1978, to be open, thus providing an improper barrier to containment leakage.
