.N011CE OF VIOLATION Vermont Yankee Nuclear Power Corporation Docket No. 50-271 Based on the results of an NRC inspection conducted on November 28 -
December 1,1978, it appears that one of your activities was not con-ducted in full compliance with the conditions of your NRC Facility License No. DPR-28, as indicated below.
This item is an infraction.
Technical Specification 6.5.B.1 states in part: "In lieu of the
" control device" or " alarm signal" required by paragraph 20.203(c)(2).
each high radiation area in which the intensity of radiation is 1000 mrem /hr or less shall be barricaded and conspicuously posted as a high radiation area... The above procedure shall also apply to each high radiation area in which the intensity of radiation is greater than 1000 mrem /hr.
In addition, locked doors shall be provided to prevent unauthorized entry into such areas."
Contrary to the above, at approximately 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br /> on November 29, 1978, Door #4, located on the 280 ft. level of the Reactor Building, and providing access to the Reactor Water Cleanup Regenerative and Non-regenerative Heat Exchanger Room, was found to be open.
The area radiation levels in this room were determined by a licensee dose rate survey to be up to 2000 mrem /hr.
