Subject:

Response to I&E Inspection Report No. 78-26 This letter is written in response to Reference b) which indicates that one of our activities was not conducted in full compliance with Nuclear Regulatory requirements. This alleged infraction was noted during an inspection conducted on November 28-December 1,1978, at the Vermont Yankee Nuclear Power Station in Vernon, Vermont.

Information is sub-mitted as follows in answer to the alleged infraction contained in the enclosure to your letter:

Item -

Technical Specification 6.5.B.1 states in part:

"In lieu of the " control device" or " alarm signal" required by paragraph 20.203(c)(2), each high radiation area in which the intensity of radiation is 1000 mrem /hr or less shall be barricaded and conspicuously posted as a high radiation area... The above procedure shall also apply to each high radiation area in which the intensity of radiation is greater than 1000 mrem /hr.

In addition, locked doors shall be provided to prevent unauthorized entry into such areas."

Contrary to the above, at approximately 1800 hours0.0208 days <br />0.5 hours <br />0.00298 weeks <br />6.849e-4 months <br /> on November 29, 1978, Door #4, located on the 280 ft. level of the Reactor Building, and providing access to the Reactor Water Cleanup Regenerative and Nonregenerative Heat Exchanger Room, was found to be open. The area radiation levels in this room were determined by a licensee dose rate survey to be up to 2000 mrem /hr.

Response -

A.P. 0150 " Responsibilities and Authorities of Operations Department Personnel" has been revised to require a check of each door once per shift to insure these doors remain locked.

In addition, step-off pads have been relocated to afford more convenient access to these doors.

Self-closing door mechanisms are also under investigation to determine if such devices are reliable and practical for this application.

Installation will be dependent upon the results of this investigation.
