7 's APPENDIX A NOTICE OF VIOLATION f
License No. R-120 North Carolina State University Pulstar Based on the NRC inspection March 13-16, 1979, certain of your activities were apparently not conducted in full compliance with NRC requirements as indicated below.
These items have been categorized as described in corre-spondence to you dated December 31, 1974.
As required by 10 CFR 21.21, appropriate procedures shall be developed A.
to assure that a responsible of ficer is informed if the construction or operation of a facility or basic component supplied for such facility:
(1) fails to comply with the Atomic Energy Act of 1954 as amended or any applicable rule, regulation, order or license of the Commission relating to a substantial safety hazard, or (2) contains a defect.
Contrary to the above, on March 13, 1979, no procedure (s) had been responsible officer of the developed or implemented to assure that a
licensee is informed of a defect or failure of a basic component which could create a substantial safety hazard.
This is an infraction.
B.
As required by paragraph 6.3.b of the Technical Specifications, temporary changes to the procedures that do not change their original intent may All be made with the apprcval of the Nuclear Operations Administrator.
such temporary changes to the procedures shall be documented and rubse-quently reviewed by the Radiation Protection Council.
Contrary to the above, during the first quarter of 1979 a temporary change concerning adjustment of compensation voltage during reactor startup was placed in effect without written approval of the Nuclear Radiation Operations Administrator, proper documentation and subsequent Protection Council review.
This is a deficiency.
