APPENDIX A NOTICE OF VIOLATION Philadelphia Electric Company Docket No. 50-277 This refers to the inspection conducted by representatives of the Regior I (Philadelphia) office at the Peach Bottom Atomic Power Station (Unit 2), Celta, Pennsylvania, of activities authorized by NRC License No. DPR-44.
During this inspection conducted on September 19-22 and October 4-6, 1978, the following apparent items of noncompliance were identified.
A.
10 CFR 50, Appendix B, Criterion IX, states, in part, that:
" Measures shall be established to assure that special processes, including
... nondestructive testing, are controlled and accomplished by qualified personnel using qualified procedures in accordance with applicable codes... and other special requirements."
Contrary to the above, on September 21, 1978, radiographs of Nuclear Class 1 pipe welds were being processed without written procedures to assure quality of final radicgraphs.
This item is an infraction.
B.
10 CFR 50, Appendix B, Criterion V, states, in part:
" Activities affecting quality... shall be accomplished in accordance with these instructions, procedures, or drawings... "
Technical Specification 6.0 requires that written procedures and administrative policies shall be established, implemented and maintained.
Philadelphia Electric Company Maintenance Procedure M-4.18, Revision 0, requires that all parts be secured to prevent any part from dropping into the reactor vessel.
Contrary to the above, on October 5,1978, uncontrolled and unsecured tools were observed on the service platform floor over the open reactor vessel.
