Dear Mr. Grier:

Thic letter responds to the infraction uncovered by Mr. L.

Narrow during his inspection of November 13-16, 1978.

Infraction 10 CFR 50 Appendix B Criterion V, states, in part, that

" Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings. . . .

and shall be accomplished in accordance with these in-structions, drawings and procedures. . ."

The licensee's Quality Assurance Plan for Forked River Nuclear Station Unit I, states, in part, in Paragraph 12.0 that:

"GPUSC requires its contractors and subcontracters to establish, document and implement a QA program for identifi-;ation, calibration and control of measuring and test equipment in accordance with the applicable specifications, codes or referenced technical documents".

Mr. Boyce H. Grier Page Two Morrison-Knudsen's Procedure GEN-ll states, in part, in Paragraphs 11.3.3 and 11.4.4 that:

"Only properly identified and calibrated tools are issued for quality controlled construction, inspec-tion and tests", and, "The calibration agency shall affix a label on each tool indicating date calibrated, due date, and facility name or logo".

Contrary to the above, on November 14, 1978, certain measuring and test equipment in use by M-K was found to be not properly identified or controlled.

Corrective Action Taken The equipment in question was re-calibrated and found to be in-calibration, thus validating the test data taken following the expiration of the date posted on the calibration-due label.

Corrective Steps to Avoid Further Infractions Copies of the Non-C' arc _;.9nce Report filed against this in-fraction were distributr.d to all f M-K's personnel involved with the use of test equipmeat and those involved with the logging and maintenance of " Calibration Due Logs" to preclude reoccurrence.

GPu-QA will conduct inspections, at scheduled intervals, of M-K's Calibration Due IJgs to insure that the status of test equip-ment in use by M-K is being monitored by M-K satisfactorily.
