  • TOLEDO EDISON COMPANY DAVIS-BESSE NUCLEAR POWER STATION UNIT ONE SUPPLEMENTAL INFORMATION FOR LER NP-33-79-63 DATE OF EVENT: May 19, 1979 EACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Violation of the integrity of a fire barrier ,

Conditions Prior to Occurrence: The unit was in Mode 5, with Power (MWT) = 0, and Load (Gross MWE) = 0. Description of Occurrence: On May 19, 1979, at approximately 0500 hours, a fire barrier above door 309 was removed to allow for removal of a hydraulic snubber SR 43 which had to be functionally tested per ST 5044.01, " Inspection Testing of Hydraulic Snubbers". Technical Specification 3.7.10 requires all fire barriers be intact at all times or a continuous fire watch be established within one hour after a fire barrier has been violated. Since the barrier was initially considered to be only a negative pressure boundary and the unit was in Mode 5, nn fire watch was established. Designation of Apparent Cause of Occurrence: The cause of the occurrence is due to faulty procedures. The job was completed ahead of schedule prior to determining if the barrier was a fire barrier or negative pressure boundary. The barrier was initially considered as a negative pressure boundary which is not required in Mode 5. Analysis of Occurrence: There was no danger to the health and safety of the public or to station personnel. There are no cable trays or combustibles in the immediate area. There was no open flame, welding, cutting or burning activities going on in the area during the time the barrier was lef t open or unattended. Corrective Action: At 0830 hours on May 19, 1979 when it was realized that the barrier had been removed ahead of schedule, it was discovered that it was a fire barrier. Personnel were immediately sent to the area and the barrier was temporarily sealed by 0900 hours on May 19, 1979. After the snubber is reinstalled, the permanent fire barrier will be restored. A modification has been prepared to ST 5344.01, " Inspection and Testing of Hydraulic Snubbers" to specify actions to be taken if the fire barrier has to be removed again. A memo will also be issued to all maintenance staf f members on required actions needed prior to violating any firewalls or negative pressure boundaries. Maintenance personnel are reviewing the administrative controls for fire barriers and negative pressure boundaries to determine if further controls are required to prevent recurrence.
