SUBJECT:

CPPR-96 INSPECTION NO. 73-13 DOCKET NO. 50-363 This letter responds to the subject inspection report which presents an infraction uncovered by Mr. A. A. Varela during his inspection October 10-13, 1978.

Below is our response to the infraction cited in the subject inspection report.

Infraction 10 CFR 50, Appendix B, Criterion V, states, in'part, that:

" Activities affecting quality shall be prescribed by documented instructions, procedures or drawings and shall be accomplished in accordance with these instructions, drawings and procedures.

Instructions, procedures or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

The licensee's Quality Assurance Plan, Revision 9, Section 5.0 states in part, the Forked River 1 Quality Assurance Program requires that:

"The basic requirements for procedures and instructions are as follows:

Appropriate qualitative and qua*ititative acceptance criteria shall be provided to ascertain that quality related activities are properly performed.

The procedures and instructions shall identify appli-cable prerequisites; ba sufficiently clear and definitive to describe the activity or operation to be performed, type of characteristics to be measured or observed, and identify requirements for documentation of results.

Quality related activities shall be accomplished in accordance with such documents."

The Morrison-Knudsen Quality Control Manual, iss.e of June 2,1978, Section 4, states, in part, that:

" Instructions, procedures and drawings shall include the appropriate quantitative and qualitative acceptance criteria to verify that important activities have been satisfactorily accomplished."

"Each Quality Engineer is responsible for ascertaining that all work within his assignment is performed in accordance with the latest revision of the project specifications and drawings."

Contrary to the above, as of October 10, 1973, the Morrison-Knudsen Construction / Quality Control Procedures CP-04-FR, Construction of Gontainment Building and CP-11-FR, Placement and Curing of Concrete did not include appropriate quantitative and qualitative acceptance criteria to verify that adequate preparations had been oerformed on the vertical construction joints of containment building base mat pour LSK-11.

As a result of this inadequacy, concrete placement had to be delayed by tne Construction Manager until corrections could be made on several nonconforming items which had been identified by the NRC inspector. The pour area had previously been inspected, accepted and released to construction by QC inspection.

Corrective '.ction Taken Marrison-Knudsen Procedure CP-11-FR, Placement and Curing of Concrete has been revised to include appropriate quantitative and qualitative acceptance criteria within its applicable checklist.

This procedure has been approved and implemented.

Inspectors ha/e been receiving on-the-job training from inspectors supplied by the Construction Manager in the use of interpretation of acceptance criteria.

M-K has instructed their inspectors in the use of Inspection Reports and the need for being more specific.

Inspection Reports are reviewed by M-K's Chief Inspector and QC Manager for acceptability prior to their entry into the Permanent Record System.

Corrective Steps to Avoid Further Infractions A review of all Morrison-Knudsen Construction /QC Procedures was per-formed by GPU QA personnel and M-K.

This review resulted in the revision of two additional procedures with regard to the incorporation of more detailed attributet into their checklists.

The procedures involved were CP-01-FR

" Placement and Compacti?n of Class 1 Backfill" and CP-12-FR " Mechanical Splicing of Reinforcing Bars".

The revisions to both of these procedures have been approved and implemented.

In the future, GPU QA will ensure that contractor's procedures contain checklists that have the appropriate quantitative and qualitative acceptance criteria.
