APPENDIX A

, NOTICE OF VICLATION Jersey Central Power and Light Company Docket No. 50-i53 This refers to the inspection conducted by representatives of the Region I (Philadelphia) Office at the Forked River Nuclear Station, Unit 1, Forked River, New Jersey of activities authorized by NRC license No.

CPPR-96.

During this inspection conducted on October 10-13, 1978, the following apparent item of noncompliance was identified.

10 CFR 50, Appendix B, Criterion V, states, in part, that:

"Activi-ties affecting quality shall be prescribed by documented instructions, procedures or drawings... and shall be accomplished in accordance with these instructions, drawings and procedures.

Instructions,

procedures or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."

The licensee's Quality Assurance Plan, Revision 9, Section 5.0 states, in part, the Forked River 1 Quality Assurance Program requires that:

"The basic requirements for procedures and instructions are as follows:

Appropriate qualitative and quantitative acceptance criteria shall be provided to ascertain that quality related activities are properly performed.

The procedures and instructions shall identify applicable prerequisites; be sufficiently clear and definitive to describe the activity or coeration to be performed, type of characteristics to be measured or observed,... and identify requirements for documentation of results.

Quality related activities shall be accomplished in accordance with such documents."

The Morrison-Knudsen Quality Control Manual, issue of June 2,1978, Section 4, states, in part, that:

" Instructions, procedures and drawings shall include the appropriate quantitative and qualitative acceptance criteria to verify that important activities have been satisfactorily accomplished."

"Each Quality Engineer is respor. ible for ascertaining that all work within his assignment is performed in accord-ance with the latest revision of the project specifications and drawings."

Contrary to the above, as of October 10, 1978, the Morrfson-Knudsen construction / quality control procedures CP-04-FR, Construction of Containment Building and CP-ll-FR, Placement and Curing of Concrete did not include appropriate quantitative and qualitative acceptance criteria to verify that adequate preparations had been performed on the vertical construction joints of containment building basemat pour LSK-ll.

As a result of this inadequacy, concrete placement had to be delayed by the Construction Manager until corrections could be made on several nonconforming items which had been identified by the NRC inspector.

The pour area had previously been inspected, accepted and released to construction by QC inspection.

This item is an infraction.
