w APPENDIX A NOTICE OF VIOLATION Yankee Atomic Power Company Docket No.50-029 Based on the results of the NRC inspection conducted October 30-November 2 and November 7-9, 1978, it appears that certain of your activities were not conducted in full compliance with NRC regulations and the conditions of your license as listed below. These items are deficiencies.

A.

Technical Specif.ication 6.0 states, in part:

" Administrative Controls are the written rules, orders, instructions, procedures

... to obtain assurance or safety and quality of operation...

These controls shall be adhered to."

Plant Administrative Procedure AP-0018 states, in part, concerning closed out Bypass of Safety Functions and Jumper Control Requests:

"The Duty Shift Supervisor shall then insert the request into the closed out section of the Bypass of Safety Function and Jumper Control Log Book and update the Master Index, APF-0018.1."

Contrary to the above, on November 3,1978, the Bypass of Safety Function and Jumper Control Log Book Master Index had not been updated in that 20 requests were listed as outstanding.

Only 10 of the 20 were actually in the active section of the book.

The status of the remaining 10 could not be immediately determined, however, it was later verified that 9 had been closed out, leaving one in question.

B.

Technical Specification 6.0 states, in part:

" Administrative Controls are the written rules, orders, instructions, procedures

... to obtain assurance or safety and quality of operation...

These controls shall be adhered to."

Procedure OP-8415 Radiation Work Permit, states that a radiation work permit (RWP) is required for fuel handling operations.

RWP 974, in use on November 8, 1978, in the spent fuel storage building, identified head covers and cotton gloves as required articles of protective clothing.

Contrary to the above, at approximately 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on November 8, 1978, a member of the security force was performing his duties in the spent fuel building without gloves.

He had entered the area as authorized by RWP 974.

At approximately 1630 hours0.0189 days <br />0.453 hours <br />0.0027 weeks <br />6.20215e-4 months <br />, he was relieved by another member of the security force who commenced his duties without a head cover.

Fuel handling operations were in progress during this time.
