Results of Routine Refueling Operations Inspection Reference is made to I&E Inspection No. 50-29/78-16, which was conducted by your Mr. W. Lazarus on October 30-November 2, and November 7-9, 1978, at the Yankee Nuclear Power Station (Yankee Rowe) in Rowe, Massachusetts. The report made subsequent to that inspection identified two items which were not conducted in full compliance with NRC require-ments. Of the two items identified in Appendix A, reference (b) noted that no response is required for Item B.
With respect to the Notice of Violation Item A reads:
Technica1 Specification 6.0 states, in part:
" Administrative Controls are the written rules, orders, instructions, procedures... to obtain assurance of safety and quality of operation... These controls shall be adhered to."
Plant administrative Procedure AP-0018 states, in part, concerning closed out Bypass of Safety Functions and Jumper Control Requests:
"The Duty Shift Supervisor shall then insert the request into the closed out section of the Bypass of Safety Function and Jumper Control Log Book and update the Master Index, APF0018.1."
Contrary to the above, on November 3, 1978, the Bypass of Safety Function and Jumper Control Log Book Master Index had not been upcated in that 20 requests were listed as outstanding. Onty 10 of the 20 were actually in the active section of the book. The status of the remaining 10 could not be immediately determined; however, it was later verified that 9 had been closed out, leaving one in question.

" Rules and Practices" Part 2, Title 10 Code of Federal Regulations, we are submitting the following action taken on this item:
1.
Corrective Steps Taken and the Results Achieved The one of ten, Bypass of Safety Function and Jumper Controls, of which the status could not be i= mediately determined has been located and the Master Index is now accurate.
A special order has been issued reminding all Operations Department personnel that the Bypass of Safety Function or Jumper Control Request forms and the associated Master Index must be filled out as appro-priate when a Bypass of Safety Function or Jumper Control Request is issued or closed out.
Personnel having responsibility for removing closed out Bypass of Safety Function or Jumper Control Requests from the Bypass of Safety Function or Jumper Control Log Book have been instructed to verify that the Master Index reflects the f act that the request has been clostd out before removing the closed out request from the inactive section of the log book.
Supervisory reviews made since the occurrence have not revealed any discrepancies.

Steps Which Will Ee Taken to Avoid Further Items of Non-Ccmpliance of This Nature With regard to Item A, all corrective steps to avoid a reoccurrence were completed and reported in (1) above.

The Date When Full Compliance Will Be Achieved Full compliance was achieved on November 28, 1978.
