APPENDIX A NOTICE OF VIOLATION s
Mississippi Power and Light Company LicenseNos.[CPPR-118
-CPPR-119 Based on the results of the NRC investigation conducted on September 18-22 and on October 10-13, 1978, it appears that certain of your activities were not conducted in full compliance with NRC requirements as indicated below. These items have been categorized as described in our correspon-dence to you dated December 31, 1974.
A.
Criterion V of Appendix B to 10 CFR 50, as implemented by the PSAR, paragraph 17.5.1, requires in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures,
..... and shall be accomplished in accordance with these instructions, procedures.....".
Paragraph 6.1 of QC instruction 9.1-9 and para-graph 11.3 of Appendix 3 to the BQAM requires in part that, "A nonconformance report shall be prepared....whenever any of the following conditions apply: (1) when welds have been made that are not in accordance with the qualified welding procedure....or appli-cable job specification....;....(5) when any condition is found after the weld has been completed, examined and accepted that does not meet the ASME code requirements or the job specification require-ments".
Contrary to the above requirements, on August 30, 1978, four socket welds (weld No. 2 on drawings FSK-I-2061A-308-T, FSK-I-1061A-312-T, FSK-I-1061A-309-T, and FSK-I-1061A-310-T) were rejected, cut out, and re-welded without a nonconformance being issued.
This is an infraction.
B.
Criterion V of Appendix B to 10 CFR 50, as implemented by the PSAR, paragraph 17.5.1, requires in part that, " Activities affecting quality shall be prescribed by documented instructions, procedures,......and shall be accomplished in accordance with these instructions, proce-dures......".
For verification of the 1/16. inch end gap fit up dimension for socket welds, paragraph 8.4 of procedure WA-1 of the Bechtel Quality Assurance Manual requires that: a scribe line be applied two inches from the end of the pipe; a second scribe line be applied to the 0.D. of the fitting corresponding with the bottom of the socket; after fitup, the distance between the two lines must be 2-1/16 inch to 2-1/8 inch; and after welding, the distance must be greater than 2 inches.

Contrary to the above requirements, on September 1, 1978, the four socket welds noted in A above were fitup, re-welded, and accepted without using the 2 inch scribe line on the pipe to check fitup (inspector witness of pipe withdrawal at fitup was used in lieu of the required method).
This is an infraction.
C.
Criterion IX of Appendix B to 10 CFR 50, as implemented by paragraph 17.1.9 of the PSAR, requires in part that, " Measures shall be established to assure that special processes, including.., and nondestructive testing are controlled and accomplished by qualified procedures in accordance with applicable codes,...."
Table 3.2.1 of the PSAR establishes various subsections of section III of the ASME Boiler and Pressure Vessel Code as the applicable code for various safety related welding. Paragraph 5112 of the various subsections of section III requires in part that, "All nondestructive examinations...shall be executed in accordance with written procedures which have been proven by actual demonstration to the satisfaction of the inspector".
Contrary to the above requirements, on October 13, 1978, the licensee readily admitted that electrical penetration welds, welded to ASME Section III, had recently been UT inspected in accordance with Bechtel UT procedure UT-PE-1, 2 and that this procedure had not been demonstrated to the inspector.
In addition, records reveal that other NDE procedures (PT, RT and MT) were used on safety related piping welds prior to their demonstration date of April 1977.
