CA 50-416, 417/7o-15 IE Inspection Report 78-15 AECM-78/99 In response to your letter dated November 22, 1978 which contained IE Inspection Report 78-15, the following is submitted:
Infraction 416/417/78-15-04 (It em A of Appendix A)
Cori :t ive St eps Taken and Result s Achieved MP6L issued D6CR 1109 on 9/21/78, documenting that the four socket welds in question were rejected, cut out and reweldsi without a nonconformance being issued. As a result of D6CR lt09, Bechtel issued NCR 2930 on 9/21/78, to document this condition for the four socket velds in question. NCR 2930 was closed out on 10/3/78.
An investigation concluded that the cause of the failure to document the weld joint removal was an isolated, misunderstanding of the nonconformance reporting program.
D&CR 1109 was closed out by !T6L on 10/3/78.
Correctiv_e_ Steps Taken to Avoid Further Noncompliance All welding QC personnel were reinstructed regarding nonconformance reporting.

Corrective Steps Taken and Results Achieved Bechtel issued NCR 2928 on 9/21/78 to document the failure to use scribe lines on the four socket veld joints in question.
NCR 2928 was closed out 11/7/78. A review of welding documentation concluded that this was an isolated misinterpretation of WD-1 requirements.
Corrective Steps To Avoid Further Noncompliance All welding QC personnel were reinstructed regarding the applicable portions of WD-1.
Date to Achieve Full Compliance Full compliance was achieved 11/7/78.
Infraction 416/78-15-02 (Item C of Appendix A)
Corrective Steps Taken and Results Achieved Bechtel UT Procedure UT-PE-1, 2 has been demonstrated to the Authorized Nuclear Inspector's satisfaction. In addition, measures have been taken to assure that all Bechtel and Peabody Testing NDE Procedures, approved for use at the Grand Gulf jobsite, have been demonstrated to the ANI's satisf action. No changes to these procedures were required as a result of these demonst rat ions.
Corrective Steps to Avoid Further Noncompliance Quality Control Instruction 9.1-28 is being prepared to est ablish controls for assuring that NDE procedures and revisions to NDE procedures are demonstrated to the ANI.
